Active, methylated form of vitamin B12. Co-formulated with tirzepatide at 0.5mg/mL (1mg per 2mL vial) to support methylation pathways during dietary changes and to address fatigue commonly reported during GLP-1 titration. The B12 inclusion is compounding-specific and not present in FDA-approved tirzepatide products.
Tirzepatide
Synthetic 39-amino-acid peptide that functions as a dual agonist of the glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors. Binds both receptors with high affinity, producing synergistic effects on glucose regulation, gastric emptying, and appetite.
Acetate buffer
Maintains reconstituted solution at pH 5.0–5.5 — the range in which the peptide is most chemically stable. Degrades to water and acetate; renally cleared.
Mannitol
USP-grade bulking agent used during lyophilization. Produces a uniform cake, prevents peptide collapse during reconstitution, and has no pharmacologic activity at the doses used.
GLP-1 receptor activation
Stimulates glucose-dependent insulin secretion, suppresses glucagon, slows gastric emptying, and promotes satiety through hypothalamic pathways. Same receptor targeted by semaglutide, liraglutide, and dulaglutide.
GIP receptor activation
Unique to tirzepatide among currently approved GLP-1 class drugs. GIP receptor activation enhances adipose tissue insulin sensitivity, may counteract the nausea associated with GLP-1 monotherapy, and contributes to additional weight loss beyond GLP-1 alone.
Central appetite regulation
Both GLP-1 and GIP receptors are expressed in hypothalamic nuclei that regulate hunger and reward. Tirzepatide reduces food intake, modifies food preferences (particularly away from high-fat/high-sugar foods), and reduces "food noise" in many patients.
Glycemic control
Improves first-phase insulin secretion in response to glucose, reduces hepatic glucose production, and improves peripheral insulin sensitivity. Clinical trials show A1C reductions of 1.8 – 2.4 percentage points vs placebo in T2DM patients.
Methylation support (B12)
Methylcobalamin supports the folate and homocysteine cycles, which become more relevant during rapid body composition change. The B12 component also addresses fatigue commonly reported during GLP-1 titration.
